Optional Term Life
MetLife Insurance Company provides individuals in your firm with the opportunity to enroll in this Optional Term Life and Accidental Death and Dismemberment (AD&D) Plan. Up to a $300,000 benefit is available upon underwriting approval. An enrolled employee’s spouse may apply for insurance up to $100,000 and a dependent child can be insured for up to $10,000.
Insured participants must be actively at work to participate in the plan. Portability Conversion options are available upon termination of employment.
