it serves by focusing on improved health outcomes. The array
of plans offered by Health Net to Builders Exchange members
includes 5 HMO plans, 2
PPO plans, and 1 PPO Health Savings Accounts (HSA) plans.
With the HMO plans, your employee chooses a Primary Care Physician (PCP) and together they manage all of the subscriber’s health care needs. With the PPO plans, your employee has the freedom to visit any physician, in-network or out-of-network.
Health Net has set 5 rate tiers: If Health Net is selected
in your firms offerings, you must enroll a minimum of 60% (if
offering HMO plans), 70% (if offering PPO plans) and 75% (if
offering either standard HMO or HSA Plans with a minimum
of 6 enrolled) of all eligible employees, as listed on the
DE-6, in any combination of Health Net plans. Health Net does
have the right to rate member firms at a different tier as
determined by Health Net underwriting based upon existing experience
or the Employer Health Statement submitted upon a firm's initial
enrollment.
