How do I check to see if my doctor is a Primary Care Physician or in the network?
Primary Care Physician and Network Provider listings for Health Net, Kaiser, Blue View Vision, American Specialty Health (Chiropractic and Acupuncture) and MetLife (Dental) are available online. Visit their sites to find your doctor.

Will I receive an ID card?
Health Net, Kaiser and American Specialty Health will send cards for you and each member of your family whom you enrolled. MetLife Dental and Vision Service Plan will not. Once enrolled in dental and or vision, we will send you a new enrollment packet that includes information for using these plans.

How do I get a referral to a specialist?
If you enroll in an HMO plan and need to see a specialist, your primary care physician will select a specialist for you and arrange for the appropriate referrals. If you see a specialist without a referral, you will be responsible for payment. If you are enrolled in a PPO plan, you may self-refer to a specialist. It is best to choose a physician In-Network as the benefits are better. If you decide to go Out-of-Network, your financial responsibility may be much greater.

How do I cover family members?
When you first enroll with our plans, you may enroll your spouse, domestic partner (see glossary of terms) and/or children at that time. If you do not enroll your dependents when first eligible, you must wait for our next open enrollment (April 1 of each year).

How do I add a NEW dependent?
If you want to add a new spouse or new dependent, including newborn children and newly adopted children, you must submit an enrollment form within 30 days of the actual event (marriage date). If you do not enroll new dependents when they are first eligible, you may enroll them only during our open enrollment (April 1 of each year).

What do I do if I lose my ID card?
To replace a lost, misplaced or worn ID card, simply call your carrier's member services phone number, which unfortunately, was located on the ID card that you need to replace. Check the carrier's link on this site under the appropriate tab or call us at (707) 542-9502.

What is COBRA? What is Cal-COBRA?
COBRA is a federal law that allows employees to continue their health coverage (at their own expense) after they leave their jobs. Also if one of your dependents loses coverage, they are also eligible for COBRA. This law is in effect regardless of whether termination was voluntary or involuntary. COBRA applies to employers with 20 or more employees. Coverage may be continued for 18 months and applies to medical, chiropractic/acupuncture, dental and vision coverage. Cal-COBRA, a California state law, applies to employers with 2 to 19 employees, and offers 36 months of coverage for continuing medical, dental and vision.
